Job summary

Bristol Hospice in Bend, Oregon is seeking a compassionate Hospice Social Worker to support patients and families at the end of life. This PRN, hourly role focuses on psychosocial assessment, counseling, and coordination with the interdisciplinary team.

Qualifications

• Must have a master’s degree in social work from an accredited university.
• If you hold an undergraduate B.A. Degree in Social Work, you are eligible to apply. Pay will be based on degree type.
• Must have minimum of two (2) years of supervised experience in health care, hospice experience preferred.
• Must understand hospice philosophy, and issues of death/dying.
• Certified Hospice and Palliative Social Worker is desirable.
• Must be flexible in work hours and able to travel throughout the Bristol Hospice service area.
• Must demonstrate a willingness to maintain knowledge regarding information systems and applicable software programs

Responsibilities

• Assess the psychosocial status of patients and families; communicate findings to the nurse and interdisciplinary team.
• Provide assessments at the patient’s residence and assist when safety is an issue.
• Carry out social evaluations including family dynamics and caregiver abilities; plan interventions.
• Counsel patient and family; provide crisis intervention when necessary.
• Educate on culture-specific needs and advance directives; educate the team.
• Maintain clinical records on all patients referred to social work.
• Serve as liaison between patients, families, and community agencies.
• Develop individualized care plans with patient and family; participate in interdisciplinary meetings.
• Assist in funeral planning, financial, legal, and health care decision discussions.
