Job summary

CareOregon is seeking a Hospice Social Worker for PRN community-based care in the Portland metro area. You will conduct psychosocial evaluations, provide ongoing counseling, and coordinate referrals as part of the interdisciplinary group (IDG) to support patient plans of care.

The role requires a Master’s in social work, Oregon licensure, and hospice experience. Expect travel, flexible hours, and a strong benefits package through CareOregon/Housecall Providers.

Qualifications

• Master’s degree in social work from an accredited college
• Current Oregon LCSW, LMSW or CSWA license
• CPR certification at hire or within 6 months in position
• Minimum one year hospice experience, including medical social work background

Responsibilities

• Perform psychosocial/bereavement evaluations of patients and families.
• Develop and implement the plan of care with IDG input.
• Assist with goals of care and POLST/advance directives as needed.
• Provide counseling and education considering family dynamics and coping methods.
• Coordinate with hospitals and facilities to promote referrals.
• Organize transfers/discharges and bereavement plans for one year post death.
• Document in EMR ensuring compliance with regulations.
