Job summary

Bristol Hospice - Bend Llc seeks a compassionate Hospice Social Worker to support patients and families at the end of life across Central Oregon. You will assess psychosocial needs, coordinate care with the interdisciplinary team, educate families on directives, and assist with funeral, financial, and legal planning.

The role requires a master’s degree in social work, at least two years of supervised healthcare experience, and willingness to travel within the Bristol service area.

Qualifications

• Must have a master’s degree in social work from an accredited university.
• If you hold an undergraduate B.A, Degree in Social Work, you are eligible to apply.
• Minimum of two (2) years of documented supervised experience in health care; hospice experience preferred.
• Certified Hospice and Palliative Social Worker is desirable.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers related to the terminal illness and communicate findings to the nurse and interdisciplinary team.
• Carry out social evaluations including family dynamics and caregiver abilities; plan intervention.
• Counsel patients and families to manage stress and coping; provide crisis intervention when needed.
• Educate interdisciplinary team on culture-related needs, communication, family roles, and traditions.
• Maintain clinical records for referred patients and assist in advance care planning.
