Job summary

Transitions Group LLC in Indianapolis is seeking a licensed social worker to join transitions hospice team on a PRN basis. You will conduct psychosocial assessments, provide ongoing emotional support to patients, families and caregivers, and connect them with community resources in home settings.

Guiding advanced directives, assisting with facility placements, and maintaining compliant documentation are key parts of the role.

Qualifications

• Master’s degree in social work from a CSWE-accredited program.
• Licensure and/or certification as required by state.
• Minimum of 2+ years social service experience in health care or social services.
• Hospice or palliative care experience preferred.
• Comfort with grief, loss, and end-of-life conversations.
• Strong knowledge of community resources and care coordination.
• Excellent documentation skills and compliance awareness.
• Self-directed with ability to work with little supervision.
• Valid driver’s license and willingness to travel locally.

Responsibilities

• Conduct initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers.
• Provide ongoing emotional support and counseling throughout the care journey.
• Identify and connect families with community resources, including financial assistance.
• Guide families through Advanced Directives with clarity and sensitivity.
• Assist with facility placement for patients when appropriate.
• Maintain accurate clinical documentation at the patient's residence and in agency records.
• Actively contribute to interdisciplinary team and IDG meetings to shape holistic care plans.
• Participate in on-call duties evenings and weekends as needed.
• Manage caseload while ensuring compliance with local, state, and federal laws, Medicare regulations, and policies.
