Pathways Hospice & Transition Home Healthcare is seeking a licensed social worker to support hospice patients and families. You will assess needs, coordinate care with nursing, and guide families through advance directives as part of a compassionate team.

The role requires Texas licensure, a social work degree, and at least two years of supervisory hospice experience, along with strong organizational and communication skills. PRN shifts with local coverage.

Qualifications

• Licensed Social Worker in the State of Texas.
• Bachelor's or Master’s degree in Social Work.
• Minimum two years of supervisory social work experience in hospice or related field.
• Excellent organizational skills and tactful interaction with diverse stakeholders.
• Proficient in Word, Excel and EHR systems.
• In good standing with Federal and State government.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct the hospice social services program.
• Interview residents and families during admission to assess needs and establish rapport.
• Explain and obtain Advanced Directives with residents and families.
• Collaborate with Nursing to ensure Do Not Resuscitate orders are accurate.
• Develop and implement social/psychosocial policies and procedures.
• Apply social work theory and professional skills to enhance patient functioning.
