Job summary

Transitions Hospice - Illinois is seeking a Licensed Social Worker to provide psychosocial support to patients and families in Winnebago, Boone, McHenry, Lake County and surrounding areas. This part-time position offers Mon-Fri, 8-hour shifts, and a salary range of $35.00 - $38.00 per hour.

Qualifications

• Master’s degree from CSWE-accredited program required in states as specified.
• Licensure and/or certification as required by the State.
• Minimum of 2+ years social service experience in a health care setting or social services agency.

Responsibilities

• Conduct thorough psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers.
• Provide ongoing emotional support and counseling as a consistent presence throughout the care journey.
• Identify and connect families with community resources and financial assistance to meet practical and emotional needs.
• Guide families through Advanced Directives with clarity and sensitivity.
• Assist with facility placement for patients when appropriate.
• Maintain accurate, timely clinical documentation at the patient's residence and in agency records.
• Actively contribute to interdisciplinary team and IDG meetings to shape holistic care plans.
• Participate in on-call duties on evenings and weekends as needed.
• Manage caseload while assuring compliance with laws and Medicare regulations.

What You'll Do

As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:

What You Bring

• Master’s degree from school accredited by the Council on Social Work Education required in states as specified.
• Licensure and/or certification as required by the State.
• Minimum of 2+ years social service experience in a health care setting or social services agency.
• Experience in hospice, palliative care, or a related healthcare setting preferred
• Deep comfort with grief, loss, and end-of-life conversations
• Strong knowledge of community resources and care coordination
• Excellent documentation skills and attention to compliance
• Self-directed with the ability to work with little supervision
• Valid driver’s license and willingness to travel locally for patient visits
