Transitions Hospice is hiring a Licensed Social Worker to support patients and families in Indianapolis and surrounding areas. You will maintain a meaningful caseload, provide grief support, and help families access community resources and advance care planning.

This role champions psychosocial care as a core element of hospice, with on-call duties evenings and weekends. A Master’s degree in Social Work and state licensure are required.

Competenze

• Master’s degree in Social Work required.
• State licensure/certification required.
• Minimum of 2+ years social service experience in healthcare or related setting appreciated.

Mansioni

• Conduct initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers.
• Provide ongoing emotional support and counseling throughout the care journey.
• Identify and connect families with community resources and financial assistance.
• Guide families through Advanced Directives with clarity and sensitivity.
• Assist with facility placement for patients when appropriate.
• Maintain accurate clinical documentation at the patient’s residence and in agency records.
• Actively participate in interdisciplinary team and IDG meetings to shape care plans.
• Participate in on-call duties evenings and weekends.
• Ensure compliance with local, state, and federal laws, Medicare regulations, and agency policies.

Formazione

Master’s degree in Social Work (CSWE accredited)
