Job summary

Everwind Hospice in Billings, MT seeks a PRN Hospice Social Worker (LCSW/LMSW) to provide psychosocial support to patients and families, assess needs, counsel, educate, and connect with community resources as part of the interdisciplinary team.

You will advocate for patient-centered care, assist with advance care planning, and document in the EMR in compliance with agency policies. Flexible PRN schedule offers meaningful work building a new program.

Qualifications

• Master's Degree in Social Work (MSW) from an accredited program required.
• Current Montana LCSW or LMSW eligible to practice independently within hospice setting.
• Hospice, palliative care, home health, healthcare, or medical social work experience preferred.
• Excellent communication, counseling, and interpersonal skills.
• Ability to work independently while collaborating within an interdisciplinary team.
• Strong organizational, documentation, and critical thinking skills.
• Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Complete psychosocial assessments and develop individualized plans of care.
• Provide emotional, psychological, and supportive counseling to patients, families, and caregivers.
• Assist patients and families with advance care planning and end-of-life decision-making.
• Identify and address social determinants of health that may impact care.
• Provide crisis intervention and emotional support during change or loss.
• Collaborate with physicians, nurses, chaplains, aides, volunteers, and the team.
• Educate patients and families regarding hospice services, coping strategies, and bereavement resources.
• Maintain timely documentation in the EMR per agency policies.
