Job summary

Luminary Hospice is seeking a PRN Master of Social Worker to implement standards of care for medical social work services within our end-of-life program. You will assess psychosocial needs, coordinate with the interdisciplinary team, and support patients and families in planning advanced directives.

This role emphasizes compassionate communication, cultural sensitivity, and adherence to regulatory requirements. PRN status implies flexible scheduling to meet patient needs and organizational goals.

Qualifications

• MSW accredited by CSWE required.
• BSW accredited by CSWE required.
• Minimum of one year healthcare or hospice experience; understands hospice philosophy.

Responsibilities

• Assesses the psychosocial status of patients and families/caregivers and communicates findings to the RN and interdisciplinary team.
• Carries out social evaluations, counsels patients and families, and provides crisis intervention when necessary.
• Educates interdisciplinary groups on cultural needs and traditions related to the patient.
• Maintains clinical records on all patients referred to social work.
• Assists in preparing advanced directives and provides information/referral services.
• Serves as liaison between patients/families and community agencies.
• Maintains collaborative relationships to support patient care and develops contracts with agencies.
