Job summary

Luminary Hospice is seeking a PRN Masters of Social Work professional to implement standards of care for medical social work services, in alignment with the service or employment agreement. This role supports patients and families at end-of-life and collaborates with the interdisciplinary team to ensure compassionate, comprehensive care.

Qualifications

• Must be a graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education.
• Or a graduate of a bachelor’s program in social work accredited by the CSWE with stated qualifications.

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the care team.
• Conduct social evaluations including family dynamics and caregiver abilities; provide crisis intervention when needed.
• Educate interdisciplinary groups on cultural needs and traditions of patients and families.
• Maintain clinical records for referred patients.
• Educate patients and families on advanced directives and available resources.
• Serve as liaison between patients/families and community agencies.
• Develop and maintain collaborative relationships with organization personnel.
• Participate in development of individualized plans of care with patient and family.
