Job summary

UnitedHealth Group seeks an RN Case Manager for in-home hospice visits across Dallas and surrounding counties. This PRN role requires Texas RN licensure, a valid driver's license, and 1+ year of clinical experience.

You will assess patients, develop and implement Plans of Care, and collaborate with the IDG to deliver compassionate, quality hospice care in home settings. Travel throughout Dallas, Rockwall, Kaufman, Hunt, and Collin counties with occasional on-call rotations.

Qualifications

• Current Texas RN license required.
• Current driver's license with access to a vehicle or reliable public transport.
• 1+ year of clinical experience in home health or hospice preferred.

Responsibilities

• Makes initial nursing evaluation within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and re-assesses every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family responses.
• Collaborates with patient/family, attending physician, and IDG to provide daily care.
