Job summary

Top Echelon Software, LLC is seeking a dedicated Hospice RN Case Manager in the Dallas area south of Downtown Dallas. This full-time role coordinates and oversees patient care, develops plans of care with the interdisciplinary team, and provides clinical assessment and symptom management.

You will educate patients and families, document accurately, and collaborate with leadership and care teams. Requirements include an active RN license (Missouri), CPR, a valid driver's license, and hospice

Qualifications

• Current RN license valid for practice (Missouri).
• Current driver's license.
• Current CPR certification.
• Two years of RN experience in a clinical care setting.
• One year of full-time hospice experience preferred.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Perform head-to-toe assessments, monitor symptoms, and manage medications.
• Educate patients and families regarding condition, needs, and care.
• Communicate patient progress with clinical leadership, physicians, and the care team.
• Complete all required documentation accurately and on time.
