Job summary

Hospice Care Partners, LLC in Irving, TX, seeks an experienced Hospice RN Case Manager to lead the plan of care for a caseload in the South Dallas area, visiting patients in homes, assisted living communities, and nursing facilities. You will report to the Director of Nursing and collaborate with the medical team to ensure compassionate, comprehensive care.

The role requires an active Texas RN license and prior hospice/home health exposure.

Qualifications

• Active Texas or compact multistate RN license in good standing.
• Hospice or home health experience preferred; strong med-surg or acute care experience considered.
• Ability to work independently in the field with sound clinical judgment.
• Reliable transportation and valid driver's license with current auto insurance.

Responsibilities

• Lead plan of care for a caseload of hospice patients in the South Dallas area.
• Complete admissions and assessments and update care plans as needed.
• Case manage symptom control and care plan updates during visits.
• Educate patients and families through the end-of-life process.
• Coordinate care with IDT and facility staff; document in EMR.
• Participate in on-call coverage on weekends on a rotating basis.
