Job summary

Accounting Search is seeking a Hospice RN Case Manager to coordinate comprehensive hospice care for patients in the Dallas area. The role involves developing individualized care plans, performing thorough clinical assessments, and collaborating with an interdisciplinary team to support patients and families.

The ideal candidate will have an active RN license, CPR certification, a valid driver’s license, and 2+ years of RN experience in a clinical setting, with hospice experience preferred.

Qualifications

• Active Registered Nurse license
• CPR certification
• Valid driver's license
• Two years of RN experience in a clinical setting
• One year of full-time hospice experience preferred
• Reliable transportation and automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care
• Develop and implement individualized plans of care with the interdisciplinary team
• Perform head-to-toe assessments, monitor symptoms, and manage medications
• Educate patients and families regarding condition, needs, and care
• Communicate patient progress with clinical leadership, physicians, and the care team
• Complete all required documentation accurately and on time
