Job summary

Suncoast Hospice, a member of Empath Health, seeks an RN Case Manager to deliver compassionate end-of-life care in Pinellas County.

The role works Monday through Friday, 8am to 5pm, under physician direction, coordinating symptom relief, assessments, and care planning with the Interdisciplinary Group to support patients and families.

Qualifications

• Graduate of approved nursing program and current unrestricted Florida RN license.
• Valid and current CPR certification required.
• Minimum one year experience in an acute-care Hospital environment; hospice RN preferred.

Responsibilities

• Under physician direction, manages symptoms and provides relief using specialized nursing skills in palliative and end-of-life care.
• Initial and ongoing assessment of patient needs across physical, functional, psychosocial, and environment aspects.
• Collaborate with the Interdisciplinary Group to develop and update the care plan and coordinate referrals as needed.
• Educate patient, family and caregivers about disease process, palliative interventions, and safety practices.

Suncoast Hospice, a member of Empath Health is seeking a caring and motivated RN Case Manager to work with us in Pinellas county while providing exceptional care on the following schedule: Monday-Friday 8-5.

What You'll Do

• Under the direction of a licensed physician, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Provides initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial, and environmental needs and activities of daily living.
• In collaboration with the other members of the Interdisciplinary Group (IDG); develops implements and updates the individualized plan of care, initiates appropriate preventive and rehabilitative procedures and initiates referrals to other services; requests complimentary services, as needed.
• Educates patient/resident, family and caregivers regarding disease process and decline, palliative interventions care giving, dying process and safety practices.

What You'll Need
