Emerald Coast Hospice is seeking an RN Case Manager to lead patient care in home settings across Florida. You will perform comprehensive assessments, implement the plan of care, and collaborate with physicians, social workers, chaplains, and other team members to ensure compassionate end-of-life support.

You will educate and support patients and families, coordinate visits, document care precisely, and contribute to quality improvement while maintaining safety and regulatory compliance.

Qualifications

• Current Registered Nurse license in the state of employment.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Coordinate with interdisciplinary team to develop and update care plans.
• Educate patients, families, and caregivers about disease processes, end-of-life care, and medications.
