Job summary

Trustbridge Hospice, a member of Empath Health, seeks an RN Hospice Case Manager in Palm Beach County to coordinate patient and family care within an interdisciplinary team. PRN hours are 8:30am–5pm with weekend availability.

The role requires an active Florida RN license, BLS, and hospice knowledge. BSN and hospice certification are preferred; EMR experience is beneficial, as is reliable transportation.

Qualifications

• RN licensed in Florida.
• BSN preferred.
• Hospice certification preferred.
• Current Basic Life Support Certification (BLS).
• Understanding of hospice nursing: case management, crisis intervention, teaching, thanatology.

Responsibilities

• Coordinate patient/family care under direction of the primary physician and RN.
• Identify patient/family needs and contribute to an individualized plan of care.
• Work within the interdisciplinary group to implement the care plan.
• Utilize monitoring and teaching to maximize comfort and quality of life for patients and families.
• Maintain confidentiality and address concerns promptly.
