Altus Hospice Care is seeking a compassionate PRN Hospice RN Case Manager to deliver hands-on nursing care and coordinate services in Corpus Christi and surrounding areas. You will guide patients and families through the hospice journey, coordinating with the interdisciplinary team to ensure comfort and quality of life.

The role requires an active RN license, at least 1 year of relevant RN experience (hospice experience a plus), and proficiency with EMR systems.

Formación

• Must hold current RN license.
• Minimum of 1 year RN experience in medical-surgical/acute care; hospice a plus.
• Computer proficiency including EMR systems.
• Current CPR certification if required by state.
• Valid driver's license and reliable transportation.

Responsabilidades

• Provide hands-on hospice nursing care and coordinate services.
• Work with interdisciplinary team to plan and adjust patient care.
• Educate and support patients and families throughout the hospice journey.
• Deliver compassionate, evidence-based nursing care to relieve symptoms.
• Assist with care planning and end-of-life comfort measures.
• Share on-call responsibilities as part of the team.
