Job summary

Reliant at Home is seeking a PRN Hospice RN Case Manager to cover Collin County and surrounding areas. The role includes comprehensive assessments, care planning, and patient-family education, with on-call responsibilities as needed.

Qualified candidates hold a current RN license and CPR, with 2 years of RN experience in a clinical setting; hospice experience is preferred. Texas licensure and reliable transportation are expected.

Qualifications

• Current RN license and ability to practice in Texas per NLC rules.
• Two years’ RN clinical care experience; hospice preferred.
• CPR certification; good interpersonal and communication skills.
• Read, write and comprehend English.

Responsibilities

• Perform initial and ongoing assessments of patient needs and determine level of care.
• Develop and implement the plan of care to ensure quality and continuity of hospice care.
• Educate patient and family on disease process, end-of-life care, nutrition, and home care.
• Provide clinical directions to Hospice Aide and LPN/LVN; document progress.
• Coordinate with physicians and IDG; on-call coverage as assigned.
