Job summary

As part of a multi-site Home Health, Hospice, Caregivers, and Rehab company, you will assess patients, coordinate care, and educate families while collaborating with therapists and physicians to ensure quality home-based care.

Qualifications

• Active RN license in Texas.
• Experience coordinating patient care in home settings is a plus.
• Knowledge of Home Health processes and multidisciplinary collaboration.

Responsibilities

• Assess patient's condition during visits and document findings.
• Perform evaluation tasks including vital signs and medication review.
• Administer medication as prescribed by the physician.
• Dress wounds and monitor healing progress.
• Educate patients and families on home health care procedures.
• Coordinate with therapists, physicians, and other care providers.
• Provide palliative care to keep the patient comfortable.
• Suggest devices or tools to improve quality of life.
• Listen to patient concerns and address requests.
