Job summary

Reliant at Home is seeking a PRN Hospice RN Case Manager to cover The Woodlands and surrounding areas. Hospice experience preferred. Home Care Home Base EMR experience is a plus.

The role requires graduate RN, active license, CPR, and reliable transportation. Prior hospice or home health experience is a plus as responsibilities include comprehensive assessments, care planning, and coordination with the IDG.

Qualifications

• Graduate of an accredited diploma, associate or baccalaureate nursing program.
• Current RN license and a valid Driver's License.
• Two years of RN experience in a clinical setting preferred; hospice experience preferred.
• CPR certification required; must read, write and comprehend English.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs in hospice care.
• Determine admission and re-certification eligibility for hospice services.
• Develop and update the care plan to ensure quality and continuity of care.
