Job summary

LiveWell is seeking a compassionate Hospice Registered Nurse (RN) to coordinate and deliver high-quality care to hospice patients in their homes. The RN Case Manager will work with an interdisciplinary team to execute the plan of care with excellence and dignity, ensuring skilled nursing care and patient-family support.

The role requires clinical management from admission to discharge, education for patients and families, and supervision of LPNs and aides, with participation in on-call rotations

Qualifications

• Minimum of 2 years of nursing experience in hospice, palliative, acute, or post-acute care settings.
• Active state Registered Nurse licensure.
• Knowledge of hospice regulations, symptom management, and end-of-life best practices.
• Valid CPR certification.
• Valid Driver’s License and auto insurance coverage.

Responsibilities

• Coordinate clinical management of assigned hospice patients from admission to discharge.
• Develop and maintain an individualized Plan of Care (POC) with the interdisciplinary team.
• Provide direct nursing care including symptom management and pain control.
• Educate patients and families about disease, medications, and end-of-life expectations.
• Lead weekly care team updates and interdisciplinary conferences.
• Communicate with physicians, social workers, chaplains, aides, and others to ensure care continuity.
• Monitor lab results and medications and update the POC accordingly.
• Supervise LPNs and aides with periodic visits.
• Provide guidance to families on hospice goals and bereavement resources.
• Document care in the EMR accurately and timely.
• Participate in on-call rotation for 24/7 coverage.

Summary:

We are looking for a compassionate Registered Nurse (RN) to coordinate and deliver high-quality care to hospice patients and their families. The RN Case Manager is responsible for coordinating and providing skilled nursing care to hospice patients in their place of residence, ensuring the plan of care is executed with excellence and dignity. This role is essential in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their healthcare journey. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Primary Responsibilities:

• Assume responsibility for the clinical management of assigned hospice patients from admission to discharge, including continuous assessment and monitoring of patient status.
• Develop and maintain an individualized Plan of Care (POC) in collaboration with the physician and interdisciplinary team, ensuring it reflects patient/family needs, goals, and wishes.
• Provide direct nursing care including symptom management, pain control, and crisis intervention in accordance with hospice protocols and patient-centered goals.
• Educate patients, families, and caregivers about disease process, medications, end-of-life expectations, and hospice services.
• Coordinate and lead weekly care team update meetings and interdisciplinary case conferences.
• Communicate regularly with physicians, social workers, chaplains, aides, and other team members to ensure continuity and quality of care.
• Understand payer requirements and collaborate with the team to ensure continued eligibility and authorization for hospice services.
• Monitor lab results, medications, and any significant changes in condition, taking action as appropriate and updating the POC accordingly.
• Supervise and support the care provided by Licensed Practical Nurses (LPNs), hospice aides, and other clinicians, including conducting supervisory visits every 30 days or as needed.
• Provide support and guidance to families regarding hospice goals, bereavement resources, and psychosocial needs.
• Ensure timely, accurate, and meaningful documentation of all care and interactions in the EMR system.
• Participate in the on-call rotation as reasonably assigned to ensure 24/7 clinical coverage.
• Perform other duties as assigned.
