Job summary

LiveWell Partners is seeking a compassionate Hospice Registered Nurse (RN) to coordinate and deliver high-quality care to hospice patients and their families in their homes. The RN Case Manager will oversee care from admission to discharge, ensuring the plan of care is executed with excellence and dignity within a collaborative team.

Responsibilities include developing a personalized Plan of Care, symptom management, patient education, and leading weekly interdisciplinary conferences.

Qualifications

  • Minimum 2 years of nursing experience in hospice, palliative, acute, or post-acute care.
  • Active state RN licensure and CPR certification.
  • Valid driver’s license and auto insurance.

Responsibilities

  • Manage clinical care for assigned hospice patients from admission to discharge.
  • Develop and maintain an individualized Plan of Care in collaboration with the physician and interdisciplinary team.
  • Provide direct nursing care including symptom management and pain control.
  • Educate patients, families, and caregivers about disease process, end-of-life expectations, and hospice services.
  • Coordinate and lead weekly care team update meetings and interdisciplinary case conferences.
  • Communicate with physicians, social workers, chaplains, aides, and other team members to ensure continuity and quality of care.
  • Understand payer requirements and collaborate to ensure continued eligibility and authorization for hospice services.
  • Monitor lab results, medications, and changes in condition, updating the POC as needed.
  • Supervise and support LPNs and aides, with supervisory visits every 30 days or as needed.
  • Provide support to families regarding hospice goals, bereavement resources, and psychosocial needs.
  • Ensure timely, accurate documentation of all care in the EMR; participate in 24/7 on-call rotation.

Summary:

We are looking for a compassionate Registered Nurse (RN) to coordinate and deliver high-quality care to hospice patients and their families. The RN Case Manager is responsible for coordinating and providing skilled nursing care to hospice patients in their place of residence, ensuring the plan of care is executed with excellence and dignity. This role is essential in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their healthcare journey. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Primary Responsibilities:
  • Assume responsibility for the clinical management of assigned hospice patients from admission to discharge, including continuous assessment and monitoring of patient status.
  • Develop and maintain an individualized Plan of Care (POC) in collaboration with the physician and interdisciplinary team, ensuring it reflects patient/family needs, goals, and wishes.
  • Provide direct nursing care including symptom management, pain control, and crisis intervention in accordance with hospice protocols and patient-centered goals.
  • Educate patients, families, and caregivers about disease process, medications, end-of-life expectations, and hospice services.
  • Coordinate and lead weekly care team update meetings and interdisciplinary case conferences.
  • Communicate regularly with physicians, social workers, chaplains, aides, and other team members to ensure continuity and quality of care.
  • Understand payer requirements and collaborate with the team to ensure continued eligibility and authorization for hospice services.
  • Monitor lab results, medications, and any significant changes in condition, taking action as appropriate and updating the POC accordingly.
  • Supervise and support the care provided by Licensed Practical Nurses (LPNs), hospice aides, and other clinicians, including conducting supervisory visits every 30 days or as needed.
  • Provide support and guidance to families regarding hospice goals, bereavement resources, and psychosocial needs.
  • Ensure timely, accurate, and meaningful documentation of all care and interactions in the EMR system.
  • Participate in the on-call rotation as reasonably assigned to ensure 24/7 clinical coverage.
  • Perform other duties as assigned.
Qualifications
  • Minimum of 2 years of nursing experience in hospice, palliative, acute, or a post-acute care setting.
  • Active state Registered Nurse licensure.
  • Knowledge of hospice regulations, symptom management, and end-of-life best practices.
  • Valid CPR certification.
  • Valid Driver’s License and auto insurance coverage.
