Job summary

John Paul VNA Hospice seeks a compassionate Registered Nurse (RN) Case Manager to coordinate and deliver high-quality hospice care to patients in their homes. The role focuses on executing the plan of care with dignity, coordinating with a supportive interdisciplinary team, and ensuring continuity of care across all services.

The RN will oversee clinical management, lead care team updates, and guide families through hospice goals and bereavement resources, while maintaining accurate EMR

Qualifications

• Minimum of 2 years of nursing experience in hospice, palliative, acute, or post‑acute care.
• Active state Registered Nurse licensure.
• Knowledge of hospice regulations and end‑of‑life best practices.
• CPR certification.
• Valid driver’s license and auto insurance coverage.

Responsibilities

• Coordinate and manage clinical care from admission to discharge with ongoing assessment.
• Develop individualized Plan of Care in collaboration with physicians and team.
• Provide direct nursing care including symptom management and crisis intervention.

Salary, Mileage reimbursed at $0.55/mile, and Payroll processed bi-weekly (26 pay periods).

Summary:

We are looking for a compassionate Registered Nurse (RN) to coordinate and deliver high-quality care to hospice patients and their families. The RN Case Manager is responsible for coordinating and providing skilled nursing care to hospice patients in their place of residence, ensuring the plan of care is executed with excellence and dignity. This role is essential in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their healthcare journey. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Primary Responsibilities:

• Assume responsibility for the clinical management of assigned hospice patients from admission to discharge, including continuous assessment and monitoring of patient status.
• Develop and maintain an individualized Plan of Care (POC) in collaboration with the physician and interdisciplinary team, ensuring it reflects patient/family needs, goals, and wishes.
• Provide direct nursing care including symptom management, pain control, and crisis intervention in accordance with hospice protocols and patient-centered goals.
• Educate patients, families, and caregivers about disease process, medications, end‑of‑life expectations, and hospice services.
• Coordinate and lead weekly care team update meetings and interdisciplinary case conferences.
• Communicate regularly with physicians, social workers, chaplains, aides, and other team members to ensure continuity and quality of care.
• Understand payer requirements and collaborate with the team to ensure continued eligibility and authorization for hospice services.
• Monitor lab results, medications, and any significant changes in condition, taking action as appropriate and updating the POC accordingly.
• Supervise and support the care provided by Licensed Practical Nurses (LPNs), hospice aides, and other clinicians, including conducting supervisory visits every 30 days or as needed.
• Provide support and guidance to families regarding hospice goals, bereavement resources, and psychosocial needs.
• Ensure timely, accurate, and meaningful documentation of all care and interactions in the EMR system.
• Participate in the on-call rotation as reasonably assigned to ensure 24/7 clinical coverage.
• Perform other duties as assigned.
