Job summary

Four Pillars Hospice is seeking a compassionate Registered Nurse (RN) to provide in-home hospice care, assess patients, and coordinate with the Interdisciplinary Team to ensure comfort at end of life. You will develop and update individualized plans of care, educate families, document clinical findings, and report changes to physicians while traveling to patient homes across Texas.

This role demands strong communication and adherence to hospice regulations.

Qualifications

  • Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing.
  • Current RN license in the state of practice and/or compact license eligibility.
  • Must have hospice experience.
  • Understanding of hospice regulatory requirements, documentation standards, and compliance guidelines.
  • Strong clinical, communication, and interpersonal skills.
  • Knowledge of end-of-life and palliative care.
  • Ability to read, write, and communicate effectively in English.

Responsibilities

  • Perform initial and ongoing patient assessments.

Registered Nurse (RN) – Hospice
Position Summary

The Registered Nurse (RN) provides nursing care to hospice patients in their place of residence and coordinates care with the Interdisciplinary Team (IDT), physicians, patients, families, and referral sources. The RN is responsible for assessing patient needs, implementing the plan of care, and ensuring quality end-of-life care.

Responsibilities

  • Valid auto liability insurance
