Job summary

Four Pillars Hospice is seeking a licensed social worker to join our compassionate team. You will assess psychosocial needs within five days of hospice election, coordinate with the IDG/IDT, and support families through end-of-life planning.

The role emphasizes counseling patients and families, collaboration with the care team, and adherence to hospice philosophy and continuing education requirements. Texas licensure and a valid driver's license are required.

Qualifications

• MSW or BSW with CSWE accreditation and at least 1 year of healthcare social work experience.
• Current LMSW or LBSW license issued by Texas Board.
• Current driver's license.
• One year of experience in palliative care or hospice preferred.

Responsibilities

• Completes the Comprehensive Assessment within five days of hospice election to identify psychosocial, financial, environmental, and community needs.
• Assists with advance directives, funeral planning, discharge planning, family counseling, and changes in level of care based on patient and family needs.
• Provides counseling and support to patients and families/caregivers regarding end-of-life issues, death, and grief.
• Participates in IDG/IDT meetings and contributes to the patient care planning process with the interdisciplinary team.
• Promotes the hospice philosophy and adheres to administrative policies to ensure quality of care.
• Demonstrates effective interpersonal and written communication skills.
• Maintains compliance with continuing education requirements of the hospice and licensing board.
• Maintains knowledge of public and private eligibility standards to ensure appropriate services.
• Accepts clinical assignments consistent with education and professional competence.
