Job summary

Four Pillars Hospice is seeking a compassionate Social Worker to join our mission-driven team. You will perform comprehensive psychosocial assessments, coordinate care with the IDG/IDT, and support patients and families through advance directives, counseling, and bereavement resources.

Ideal candidates hold an MSW or BSW with CSWE accreditation or a related bachelor’s degree with MSW supervision, plus healthcare social work experience. Texas licensure and a valid driver’s license are required.

Qualifications

• MSW degree from CSWE-accredited school OR BSW from CSWE-accredited institution OR Bachelor’s in psychology, sociology, or related field with MSW supervision and 1 year social work experience in healthcare.

Responsibilities

• Completes psychosocial assessments and coordinates care with IDG/IDT.
• Assists with advance directives, counseling, and family support.
• Participates in care planning and on-call rotation.
• Promotes hospice philosophy and adheres to policies.
• Maintains knowledge of eligibility standards and requirements.
• Ensures effective communication with patients, families, and team.
