Job summary

Heart to Heart Hospice is seeking a PRN Social Worker to join our compassionate team dedicated to patients with life-limiting illnesses. You will assess patient needs, provide counseling, assist with advance directives and connect families with community resources.

Requirements include a social work degree from an accredited program, an active state license, a valid driver's license, and at least one year of clinical social work experience, with hospice experience preferred.

Qualifications

• Bachelor's or Master's degree in social work from a state accredited program.
• Current state license required in accordance with State regulations.
• Current driver's license with reliable transportation and ongoing auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting; hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
