Job summary

OhioHealth at Home in partnership with Compassus is seeking a Home Health Speech Language Pathologist - PPV PRN. The role focuses on evaluating and treating speech, language, cognitive, and swallowing disorders in a home health setting under physician direction.

The SLP will assess, plan, implement, and document care, collaborate with families and care teams, and ensure quality and continuity of services according to the plan of care. Licensing and professional standards are required.

Qualifications

• Bachelor's degree in Speech Language Pathology is required.
• Master's degree in Speech Language Pathology is preferred.
• Minimum of one year of home health experience is required.

Responsibilities

• Provides care following infection control measures (OSHA).
• Maintains effective communication with patient/family and care team to coordinate services.
• Develops and updates the Plan of Care in consultation with the physician and care team.
• Provides speech-language pathology services and guidance to patients and families.
