Job summary

Compassus is seeking a Home Health Speech Language Pathologist (PPV) to provide expert evaluation and treatment for patients with speech and language challenges in a home-health setting. You will work under the Compassus philosophy and participate in interdisciplinary care to support patients and families.

The role emphasizes development of plans of care, ongoing patient assessment, documentation, and collaboration with physicians and the care team to achieve optimal outcomes.

Qualifications

• Bachelor’s degree in Speech Language Pathology is required; Master’s is preferred.
• Active Speech Language Pathologist license in state of employment required.
• Experience in home health setting is valued.

Responsibilities

• Participates in development of speech therapy plans of care after consultation with referring physician.
• Visits patients for assessment, evaluation, and completion of OASIS assessments as needed.
• Educates and instructs team members, patients, and families on care plans.
• Documents care and maintains medical records accurately and timely.
