Job summary

Frontpoint Health is seeking a Registered Nurse (RN) to provide home health care in Kansas. Ideal candidates will have a TX RN license and at least 2 years of experience in home health. Responsibilities include skilled nursing, patient assessments, and coordination of care.

This role requires reliable transportation, CPR certification, and familiarity with EMR. The RN will ensure high-quality care in patients' homes and maintain confidentiality as per HIPPA regulations.

Qualifications

• Current Texas State license as a Registered Nurse.
• 2+ years experience as a RN in Home Health.
• Current CPR certification.
• Reliable transportation.
• Texas Drivers License and Auto Liability Insurance.
• Current TB Skin Test.
• EMR experience (Kinsser preferred).

Responsibilities

• Case management and supervised visits.
• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Assess and chart observations of the patient’s condition at each visit.
• Provide skilled nursing care as needed in accordance with the plan of care.
• Provide proper patient education for home health care procedures.
• Responsible for the overall clinical record maintenance.
• Maintain confidentiality of patient PHI.
• Keep the patient comfortable through palliative care.
