Job summary

Frontpoint Health is looking for a Registered Nurse (RN) to provide home health care in Venus, TX. The RN will manage patient visits, coordinate care with an interdisciplinary team, and participate in case management.

Qualifications

• Must hold a current Texas State license as a Registered Nurse.
• Minimum of 2 years RN experience in Home Health required.
• Valid CPR certification and transportation necessary.

Responsibilities

• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Provide skilled nursing care as needed in accordance with the plan of care.
• Maintain confidentiality of patient PHI through HIPPA law.
• Responsible for the overall clinical record maintenance.
