American Medical Home Health Services is seeking a dependable Registered Nurse (RN) to provide home health visits in Northwest San Antonio and surrounding areas. The position is PRN/per-visit with flexible scheduling and competitive per-visit compensation.

Electronic documentation is done through WellSky/Kinnser, with NestMed AI available to assist with clinical documentation. Ideal candidates hold a Texas RN license, have at least one year of nursing experience, and possess CPR certification.

Qualifications

• Current and unrestricted Texas RN license.
• Minimum of one year of nursing experience.
• Home health and OASIS experience strongly preferred.
• Current CPR certification.
• Reliable transportation, valid driver’s license, and automobile insurance.
• Ability to manage visits independently and complete documentation promptly.

Responsibilities

• Complete comprehensive patient assessments.
• Perform skilled nursing visits according to the plan of care.
• Complete Start of Care, Resumption of Care, Recertification, and Discharge assessments, Supervisory, Routine visits as assigned.
• Communicate changes in the patient’s condition to physicians and the clinical team.
• Provide patient and caregiver education.
• Submit accurate and timely clinical documentation.
• Coordinate care with therapists, providers, and other members of the interdisciplinary team.
