Job summary

Dignity Care Partners Home Health IN is seeking an experienced Home Health Registered Nurse to assess, plan, and coordinate patient care in the home setting. You will support families, supervise LPNs and aides, and ensure effective care plans and education for patients navigating home-based recovery.

The role requires an active RN license, BSN preferred, and 1–2 years acute care experience with home health exposure. Must have reliable vehicle and valid driver's license.

Responsibilities

• Conduct initial assessments of patient and family to determine home care needs.
• Perform comprehensive physical assessments and review medical history.
• Supervise and direct LPN and Home Health Aide in care plan implementation.
• Develop and modify patient education plans; oversee education delivery.
• Create and adjust the care plan based on patient status.
• Use health data to determine nursing diagnoses and plan of care with goals.
• Administer medications and treatments as prescribed.
• Coordinate discharge planning and act as Case Manager as assigned.
• Prepare clinical notes and communicate patient needs to physician.

Some of your primary responsibilities will include:Completing an initial assessment of patient and family to determine home care needs.
