Job summary

Stonerise Home Health in Indianapolis, IN seeks a Home Health RN Field Nurse to plan, organize, and direct home care services with a focus on community health education. You will build on community resources to meet patient and family needs in homes and communities.

Responsibilities include initial assessments, care planning, supervising LPNs and HHAs, patient education, and coordinating discharge planning.

Qualifications

• Graduate of an accredited school of nursing.
• Active RN licensure.
• One to two years of recent acute care experience in an institutional setting.
• Bachelor’s degree with one year of home health care experience preferred.
• Must be a licensed driver with a valid automobile insurance.

Responsibilities

• Complete initial patient and family assessment to determine home care needs.
• Develop and implement patient education plans and care plans.
• Supervise LPNs and Home Health Aides and modify plans as needed.
• Coordinate discharge planning and provide case management for assigned caseload.

As a Home Health RN Field Nurse, you will plan, organize, and direct home care services with an emphasis on community health education and experience. You will build from resources of the community to plan and direct services to meet the needs of patients and families within their homes and communities. **Some of your primary responsibilities will include:*** Completing an initial assessment of patient and family to determine home care needs.
