Mission Healthcare, based in Riverside, CA, is seeking licensed registered nurses for PRN home health visits. You will perform comprehensive assessments, develop care plans, and collaborate with patients and families to meet care needs.

Required are an active RN license, BLS/CPR, 1 year of acute care experience, and a valid driver's license. Eligible employees receive a strong benefits package and flexible scheduling.

Qualifications

• Active RN license in the state.
• Minimum 1 year of acute care experience in an institutional setting.
• BLS/CPR certification required.
• Valid driver’s license and auto insurance.

Responsabilités

• Perform comprehensive patient assessments and family interviews to determine home care needs.
• Document medical history and physical assessments.
• Develop and update individualized care plans with patient/family participation.
• Identify discharge planning needs and ensure they are addressed before discharge.
• Provide nursing care and counseling to patients and families.
