Job summary

Caremed, Inc. is seeking dynamic Registered Nurses for PRN and Full-Time roles to deliver compassionate home-based care across multiple VA counties.

The RN will perform visits, assessments, education, and coordination with healthcare teams, ensuring high-quality, patient-centered care in the home setting. Ideal candidates hold an active VA RN license, ADN/BSN, and CPR/BLS certification, with at least 1 year of home health experience and a valid driver's license.

Qualifications

Active RN license in Virginia (good standing).
Graduate of an accredited nursing program (ADN or BSN).
Current CPR/BLS certification required.
Minimum 1 year RN home health experience preferred.
Valid driver’s license and reliable transportation.
Strong clinical judgement and compassionate patient-first mindset.

Responsibilities

Conduct regular home visits to assess and support patients’ needs.
Perform comprehensive physical, psycho-social, and environmental assessments at home.
Educate patients and families to improve quality of life and outcomes.
Collaborate with multidisciplinary teams to ensure seamless care delivery.
Document visits accurately and supervise home care aides as needed.
