Job summary

Mission Healthcare is seeking a Registered Nurse to join our home health team in the South West Portland area. The role includes initial patient assessments, developing and updating care plans, and collaborating with families to meet care goals.

You will provide direct patient care, administer prescribed medications, and educate patients and families. Requirements include an active state RN license, at least 1 year of acute care experience, BLS/CPR, and a valid driver’s license.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• BLS certification with hands-on CPR.
• Valid driver's license and auto insurance.
• Associate's or Bachelor’s degree in Nursing, and 1 year of home health experience preferred.

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Regularly re-evaluates patients’ needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan which establishes goals, based on nursing diagnosis, incorporates therapeutic, preventive, and rehabilitative nursing actions.
