Job summary

Alc-Health-Care-Services is seeking a PRN Physical Therapist to deliver home health PT services under a physician-approved plan of care. The role offers per-visit pay: Standard visits $75.00 and Evaluations $85.00, with flexibility in scheduling and geographic area coverage.

The position emphasizes patient assessment, treatment, discharge planning, and collaboration with home health teams. Travel is involved, with administrative support and direct communication to the Home Health Director.

Qualifications

• Requires degree in physical therapy approved by an accredited organization.
• Licensed to practice as a physical therapist in the state where applicable.
• Two years of physical therapy experience preferred; home health experience is a plus.
• Good verbal and written communication and organization skills.
• CPR certification is required.
• Must have a valid driver’s license and be insured.
• Independent general liability/workers’ comp coverage/cert is required.

Responsibilities

• Provide PT services per physician's plan of care, assess functional status, and evaluate home environment.
• Assess muscle strength, mobility, gait, ROM; determine rehab potential.
• Direct PT treatment and educate patients and families on use of therapeutic devices.
• Identify needs for other home health services and refer as needed.
• Prepare and submit clinical progress summaries and discharge plans.
• Provide PT consultation to home families/caregivers and conduct in-service education.
