Job summary

ALC Home Health is seeking a PRN Physical Therapist Assistant to deliver PT services in patients' homes, following physician plans of care and coordinating with a Home Health Director.

You will assess functional status, contribute to the total plan of care, and instruct patients and families in therapy activities. The role offers flexible scheduling, travel to patient locations, and administrative support to track visits.

Qualifications

• Graduate of an accredited physical therapy assistant program with an Associate's degree approved by the American Physical Therapy Association.
• Current license or certification as required by state law.
• Two years of PTA experience; community/home health experience preferred.
• Excellent verbal and written communication skills; organized.
• CPR certification current; valid driver’s license with insured vehicle.

Responsibilities

• Provide physical therapy services per physician's plan of care and evaluate patient progress.
• Assess strength, mobility, gait, ROM; determine rehab needs and environment suitability.
• Instruct patients and caregivers in use of therapy equipment and home exercises.
• Develop and submit progress summaries; participate in discharge planning.
• Supervise PT assistants; coordinate with home health staff and family members.
