Job summary

ALC Home Health seeks a PRN Physical Therapist to deliver care in patients\' homes across assigned routes. You will perform evaluations, develop and modify plans of care, and guide patients and families in therapy programs.

Flexible scheduling supports a mobile, community-based practice while maintaining clinical standards. The role emphasizes home health excellence, documentation, and collaboration with physicians, aides, and nursing staff.

Qualifications

• Current license to practice physical therapy in the state.
• Two years of home health experience preferred.
• CPR certification current and valid.
• Must have a valid driver's license and vehicle.

Responsibilities

• Provide PT services per physician plan of care.
• Assess functional status, mobility, ROM, gait; update plan of care.
• Instruct patients and caregivers in therapeutic exercises and devices.
• Document progress and discharge planning per policy.
• Coordinate with family and other home health staff.
