What You Will Do as a PRN Home Health Physical Therapist

As a Home Health Physical Therapist, you will be responsible for providing physical therapy services and supervising team members.

Some Of Your Primary Responsibilities Will Include
  • Providing physical therapy services to patients according to the patient plan of care, including assessment and evaluation of therapeutic, rehabilitative, and functional status, muscle strength, mobility, gait, ROM, etc.
  • Initiating physical therapy programs and instructing other team members and/or families/caregivers in therapy phases with which they may work with the patient
  • Training patients on the use of prosthetic devices
  • Identifying patient and family/caregiver needs for other home health services and referring as necessary
  • Preparing and submitting clinical and progress summaries based on patient goals
  • Participating in discharge planning of patients
  • Completing a 30-day functional reassessment and revising plan and goals accordingly
  • Providing physical therapy consultation to home families/caregivers as needed
  • Supervising and directing physical therapy assistants (PTA)

Schedule

Full Time, Pay Per Visit.

Education, Licensure & Certification Requirements
  • Possesses a baccalaureate level degree or higher in physical therapy from an accredited organization
  • Licensed to practice as a Physical Therapist within the state
  • At least two (2) years of experience as a PT. Community/home health experience preferred
  • Valid Driver’s License and a reliable, insured vehicle in good working condition
