The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist works to develop a treatment plan, performs interventions to improve and enhance the patient's well-being, and evaluates progress.

• Evaluate the patient's functional status and physical therapy needs. Assist the physician in developing the therapy plan of care.


• Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.


• Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning according to accepted standards of practice.


• Instruct and inform the healthcare team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.

Licensure Requirements

• Current Physical Therapy licensure in Indiana


• Current CPR certification


• Must have a valid driver's license, vehicle insurance, and access to a reliable vehicle
