What You Will Do as a Home Health Physical Therapist

As a Home Health Physical Therapist, you will be responsible for providing physical therapy services and supervising team members.

Some of Your Primary Responsibilities Will Include
  • Providing physical therapy services according to the patient plan of care, including assessment of therapeutic/rehabilitative/functional status, muscle strength, mobility, gait, ROM, etc.
  • Initiating physical therapy programs and instructing other team members and/or families/caregivers in therapy phases
  • Training patients on prosthetic device use
  • Identifying needs for other home health services and referring as necessary
  • Preparing and submitting clinical and progress summaries based on patient goals
  • Participating in discharge planning
  • Completing 30-day functional reassessments and revising plans accordingly
  • Providing physical therapy consultation to families/caregivers
  • Supervising and directing physical therapy assistants (PTA)

Schedule

Full Time, Pay Per Visit.

Education, Licensure & Certification Requirements
  • Baccalaureate degree or higher in physical therapy from an accredited organization
  • Licensed to practice as a Physical Therapist within the state
  • At least two (2) years of experience as a PT; community/home health experience preferred
  • Valid Driver’s License and insured vehicle in good condition
