Qualifications

Responsibilities

• Performs direct and indirect home health nursing care per agency policies.
• Develops, implements, and evaluates patient care plans with the IDT.
• Monitors and documents patient status; coordinates care with the team.
• On-call as required; ensures continuity of care and safety in home settings.
• Completes Oasis documentation per Medicare guidelines.
• Provides patient education and supports age-appropriate care across all visits.
