Job summary

SHC WVUHS Home Care in the United States is seeking an experienced registered nurse to serve as a case manager for patients at home. You will develop, implement, and evaluate nursing care in collaboration with patients, families, and the health care team.

The role requires an active RN license, CPR within 30 days, a valid driver’s license, and 1 year of home care experience along with 2 years of general nursing background.

Qualifications

• Current RN license issued by the state where services will be provided or a multi-state RN license via eNLC.
• Valid driver’s license.
• CPR certification within 30 days of hire date.
• One year of Home Care Nursing and 2 years of prior nursing experience.

Responsibilities

• Performs home health nursing care and monitors patient condition per policy.
• Maintains a safe home environment and ensures patient privacy and dignity.
• Documents patient status and care provided in appropriate records.
• Coordinates care with the interdisciplinary team to ensure continuity.
• Assists with infusion therapy, vital signs, and wound care as directed.
• Delivers patient teaching and coordinates medications as ordered.
• Completes OASIS SOC/ROC and Recertification per Medicare guidelines.
• Performs on-call duties as required and communicates care needs to staff.
