Job summary

SHC WVUHS Home Care in Morgantown, WV seeks an RN Hospice Case Manager to provide direct and indirect nursing care in patients' homes, coordinating with the IDT to develop and evaluate care plans. The role emphasizes compassionate end-of-life care and adherence to agency policies.

Responsibilities include monitoring patient status, ensuring safety, performing procedures, documenting care, and supporting on-call coverage; requires valid RN license, CPR, driver’s license, and hospice/home care

Qualifications

Current RN license in WV or eNLC multi-state license is required.
Valid driver’s license and CPR certification within 30 days of hire.
One year of Home Care Nursing experience preferred; hospice experience advantageous.

Responsibilities

Provide direct and indirect nursing care in the patient’s home under IDT guidance.
Develop, implement, and evaluate nursing care plans according to policies.
Monitor patient conditions, document status, and communicate changes to the team.
Assist with admissions, on-call duties, and coordination of care with the IDT.
Ensure safety, privacy, and dignified care across age groups and settings.
Administer medications and manage IVs, dressings, and ostomy care per protocol.
