Job summary

Haven Home Health & Hospice is seeking an In-Home RN/LPN for PRN work in the Springfield, MO area. This role focuses on delivering skilled nursing visits in clients' homes and providing teaching to patients and families.

You will coordinate day-to-day nursing services, perform assessments, and support patients to stay independent at home with a flexible PRN schedule and meaningful patient relationships.

Qualifications

• Current Missouri RN or LPN license
• Minimum one year of professional nursing experience within the last three years
• Six months of experience with Medicaid In-Home Services nursing preferred
• Geriatric nursing experience is helpful

Responsibilities

• Performs client visits as needed with authorization by the Division of Senior and Disability Services, as well as reassessment visits for In-Home Services and Advanced Personal Care (APC) clients
• Weekly medication setup
• Monthly diabetic nail care
• General health evaluations
• Skin assessments
• Nursing assessments

Skills

What You'll Do
