Job summary

Haven Home Health & Hospice is seeking an In-Home RN/LPN (PRN) to provide skilled nursing visits in the home. You will coordinate and deliver nursing services, focusing on patient independence and quality care with flexible scheduling.

The role requires current Missouri RN or LPN license and at least 1 year of nursing experience; Medicaid In-Home Services experience is a plus. Geriatric nursing experience is helpful in this environment.

Qualifications

• 1 year of professional nursing experience (within last 3 years)
• Six months Medicaid In-Home Services nursing preferred
• Geriatric nursing experience is helpful

Responsibilities

• Performs client visits in home with authorization from relevant authorities and conducts reassessment visits for In-Home Services and APC clients.
• Weekly medication setup.
• Monthly diabetic nail care.
• General health evaluations.
• Skin assessments.
• Nursing assessments

What You'll Do

As an In-Home RN/LPN, you'll participate in the day-to-day coordination and delivery of skilled nursing services while providing teaching and nursing visits that help ensure the highest quality of care.

Responsibilities include:

• Performs client visits as needed with authorization by the Division of Senior and Disability Services, as well as reassessment visits for In-Home Services and Advanced Personal Care (APC) clients.
• Weekly medication setup
• Monthly diabetic nail care
• General health evaluations
• Skin assessments
• Nursing assessments
