Job summary

Haven Home Health & Hospice in Springfield, MO is seeking a compassionate Hospice Registered Nurse to provide skilled nursing services to patients in their homes during the end-of-life process. The RN will work with families to ensure dignity, peace, and comfort, following physician orders.

The role requires a Missouri RN license; hospice or home health experience is preferred. You will collaborate with an interdisciplinary team, educate families, and document care plans and outcomes.

Qualifications

• Current Missouri Registered Nurse license in good standing.
• Preferred minimum of one year nursing experience within the last five years.
• Hospice or home health experience preferred.
• Strong communication and organizational skills.
• Proficient in computer skills and clinical documentation.
• Flexibility in work schedule to accommodate patient needs.

Responsibilities

• Conduct comprehensive nursing assessments.
• Develop and implement individualized plans of care in collaboration with physicians and the interdisciplinary team.
• Educate and support patients and families throughout the end of life process.
• Provide compassionate care focused on comfort, dignity, and quality of life.
• Supervise and support LPNs and CNAs in delivering nursing services.
• Complete accurate and timely clinical documentation.

Job Summary
